Request for Make up lesson

All make-up lesson requests must be submitted through this form. Each student can request up to 5 make-up lessons. You will be receiving an auto-reply if the submission is successful.

Each student is permitted to request up to 5 make-up lessons only.

I am responsible for attending the make-up lesson with no a reminder.

Make-up lesson is only allowed for the subsequent class only.

I may not be able to participate in a workshop due to maxium no. of people allowed.

The college has the right to reject any request if the class is full.

Students who have completed their course period cannot request for make-up lesson.
